This happens when you use the built-in image selection in the ckeditor.

"Easy" 🙂 You need a mautic v 5.x. instance. Add /elfinder?editor=ckeditor#elf_fls1_Lw after the domain. eg. mautic.example.com/elfinder?editor=ckeditor#elf_fls1_Lw
Or for the user's workflow: go to channels -> focus items -> new -> builder -> content -> editor and click on wysiwyg icon insert image or file
