2FA code not working after Windows formatting / reloggin into vault

I just formatted by Windows PC and reinstalled Firefox and all the plugins. I logged into my vault and tried to log into my Cloudron server. I have 2FA enabled for the admin account. When I enter the credentials & TOTP code, it says wrong password. I hopped on my other PC and used the 2FA from that vault, and it logs me in just fine. This issue is affecting all 2FA codes in my vault on a newly formatted machine. Anyone experienced this?
P.S. On a related note, I noticed 2FA is broken on organization (shared) logins too, but I haven't looked into it and it might be a different issue.

It was a time issue. I fixed it by selecting the right timezone and enabled "set time automatically" on the affected Windows machine.
