Standard template for App Wishlist

I am moderating the App Wishlist section of the forum. I think it would save work and help people if we created a sticky there with a suggested template for making Application Wishes there.

What info would you like to see on a standard App Wish? How would you like it structured?
To help start the discussion, here is an example:

• Licence: e.g. AGPL
• Docker: Yes/No
• Demo: link
• Summary: introduction to the application
• Notes: why you like it/concerns
