I'm looking for a great FOSS survey tool to replace an Airtable based workflow but it seems difficult. LimeSurvey has a ton of options which is great but it way, way too complex for end users. Is there anything that has a nice UX that's also open source?

The Webform module for Drupal CMS may be an alternative. Good news: you can install Drupal in the LAMP app on Cloudron.
https://www.drupal.org/project/webform To get an idea of Webform, check out https://webform.com/features.

@jdaviescoates is it still under active development? I love the UX but it seems dead

@ianhyzy I had some trouble installing it on a different VPS
            probably my idiocy
            may try again
but it slipped down my wish list
not sure about active development status : last update a few months ago doesn't seem too bad, but maybe no major new features

Maybe one of the many WP plugins like https://wordpress.org/plugins/perfect-survey/ ? (this says it's opensource as well)

If you are using Nextcloud, or don't mind installing it, you can try the Polls module:
